Causes of Hair Loss in Men

Hair loss in men is a common sight around the world. Many people do not understand that male hair loss is a medical disorder known as androgenetic alopecia.  It affects up to 60% of the male population in almost every ethnicity; it is also referred to as male patterned baldness (MPB). The Aging Process
Male pattern baldness is by far the most common cause of hair loss in men, affecting 60% of men by age 50. Male pattern baldness typically affects the frontal hairline, temporal recessions and crown. Pattern hair loss affects 1 in 5 women. In women, hair loss is usually related to hormones and typically demonstrates a more diffuse pattern.

Hair loss is primarily caused by a combination of aging, a change in hormones, and a family history of baldness. Baldness is often blamed on poor circulation to the scalp, vitamin deficiencies, dandruff, and even excessive hat-wearing.

Causes of Baldness in Men

Hormonal and genetic factors both play a role, but the exact root mechanism for baldness in men is still being researched by medical science.

Dihydrotestosterone (DHT) a powerful sex hormone is known to be the trigger for male pattern baldness. It has been noted that men with MPB have elevated levels of the enzyme 5-alpha-reductase. The act of genes, hormones, stress in life, medications and time makes prone hair shorter and finer over time and before it eventually dies out. The progress of this miniaturization causes disappearance of certain hair shafts and shrinkage of the others. In this process the scalp hair becomes very fine and short, which is also known as miniaturized hair.

Hair loss can begin as early as the end of puberty. The Hamilton-Norwood scale is used for the classification of male pattern baldness and is based on the progress and severity of hair loss. Currently, several treatment options are available. Besides hair transplantation, there are two medications that have been FDA approved for use in the United States. Patients who benefit the most from effects of medications on slowing the baldness process are still in the initial stages of baldness.

Treatment of Male Pattern Baldness (MPB)

Medical treatment for baldness in men has always been a challenge. Patients’ experience varied results with the available approved medications. Minoxidil it one of  two approved medications it comes in topical form of Minoxidil; it is the first FDA approved medication to market.  Minoxidil is available in 2%, 5%, 10% and now even 12.5%  solutions. Minoxidil needs to be applied to the balding areas of the scalp and its effectiveness is usually not noticeable for several months after initiation of treatment. Positive results of the medication are not permanent and in most cases a patient will resume hair loss after discontinuation of medication. Hair transplant surgeons may recommend use of Minoxidil treatment along with hair transplantation as part of your hair restoration.

 The only available oral medication for male pattern baldness is finasteride (Propecia). It is available for hair loss treatment in one milligram pills and one pill should be taken every day. Withdrawal usually leads to reversal of hair loss within 12 months.

 Hair Loss Causes in Women

The rate of hair growth is about 1/2 inch per month. The life of each hair includes cycles of growth (Anagen phase) and rest (Telogen phase) which for scalp hair takes around 2 to 6 years ending with hair fall out. After the falling of hair a new hair grows in its place. In a normal person, 85 percent of hair is growing at any given time and the other 15 percent is resting.

 Baldness is a condition in which new hair does not grow when old hair falls or the new hair is miniaturized. The cause of this condition in female pattern baldness is not entirely understood, but it seems to be associated with genetic, hormonal changes, aging and deficiencies. Hormonal changes are commonly seen in the menopausal period when scalp hair is thinned, while facial hair grows coarser.

Female pattern baldness is a specific pattern of hair loss observed in some women. FPB is often caused by hormones, aging etc.

Pattern and Grading of Female Hair Loss

The typical pattern in female pattern baldness is generally different from that of male pattern baldness). In female pattern baldness, the hair thins out more but the front hairline maintains its look in most cases (unlike male pattern baldness with frontal recession). Female pattern baldness can occasionally appear similar to male pattern baldness, but it rarely progresses to full or even near total baldness as it may in men.

 Some typical patterns of hair loss in women are:

  1. Diffuse pattern of hair thinning over entire scalp, often with more noticeable thinning toward the back.
  2. Diffuse thinning with more noticeable thinning in front, but with no involvement of the frontal hairline. Patients in this group have the best result from hair transplant if they have an intact donor hair.
  3. Diffuse thinning with more noticeable thinning toward the front of the scalp with involvement and sometime breaching the frontal hairline.
  4. Diffused Unpatterned Alopecia (DUPA) in which hair becomes thin diffusely without following any specific pattern. These patients are generally not good candidates for hair transplant.

 Other Causes of Hair Loss in Women

 Other than genetic and hormonal causes of hair loss in women, female hair loss can be accelerated by other factors such as:

  • Telogen effluvium- temporary shedding of hair at more than the usual rate. This is a common cause of hair loss for men and women. Accelerated hair loss after delivery of a baby is not uncommon.
  • Instrumental- breaking of hair (usually from hairstyling treatments)
  • Alopecia areata- patchy areas of total hair loss (an immune disorder causing local hair loss.)
  • Scarring alopecia-patchy hair loss due to infection, inflammation or injury, which is then followed by scarring.
  • Trichotillomania- compulsive pulling of hair due to anxiety or other psychological issues.
  • Some skin disorders such as lupus.
  • Medications: Chemotherapy mediations, Alloppurinol, Heparin, Coumarin, Clofibrate, Gemfibrozil etc.

Prior to treating hair loss, all common treatable causes have to be excluded. A simple lab test can exclude many of these disorders. Rogaine (topical Minoxidil 2%) is often the first step in medical treatment of these patients. Rogaine is the only FDA approved medication that is currently available for medical treatment for female hair loss. The other FDA approved hair loss treatment in males, Propecia, does not show any positive results in women.

 Other Types of Hair Loss.

 Alopecia Areata

Alopecia areata is an auto-immune disease that affects hair follicles anywhere in the body, appearing as a rounded patchy area of hair loss. Alopecia areata affect both men and women and could be seen almost anywhere on the skin.

Anagen Effluvium

Sudden hair loss due to the effect of chemicals or radiations such as hair loss following chemotherapy, radiation therapy or toxins is typical conditions called anagen effluvium. This condition is different from Telogen effluvium, in which hair enters the resting phase. In this condition, hair loss begins 1-3 weeks following exposure to the inducing factor. Some types of anagen effluvium such as hair loss after chemotherapy could be temporary and hair growth is back to normal once treatment is finished. The texture of hair could be somehow different after re-growth in some cases.

Trichotillomania

Trichotillomania is a self induced hair loss in which patient continuous pulling hairs cause hair loss in some areas. Emotional and psychological disorders such as anxiety are usually involved. It occurs most commonly among children and adolescents. Women are affected twice as men. It could also occur on the eyebrow, beard and body hair.

Traction Alopecia

Traction Alopecia is caused by continues pulling on the hair causing local hair loss in the affected area. Many factors could be responsible such as various hair styling, ponytails and tight braiding. The process of hair loss is gradual, but if it continues, hair loss could be permanent. Permanent types of traction alopecia could be easily treated with hair transplantation.

Telogen Effluvium

Sudden and usually stress related hair loss, which presents itself as general thinning throughout the scalp. Telogen Effluvium occurs after any sever and sudden stress. In Telogen effluvium a large number of hair follicles go to sleep phase at once causing hair loss or thinning all over or in a large area of scalp. In most cases the hair loss recovers spontaneously and completely. Telogen effluvium can be seen in patients who are prone to male or female patterned baldness and stress cause accelerate hair loss that may be not reversible due to underlying conditions. The most common form of telogen effluvium is seen after childbirth or pregnancy termination for other reasons in women.

Shock Loss after Surgery

The stress and physical changes following major surgeries or procedures of head and scalp area can cause sudden hair loss in involved areas called shock loss. Hair transplant used to be one of the major causes of shock loss, but using the medication Propecia has reduced the rate of hair loss after hair transplant surgeries significantly.

Recovery after Hair Transplants
Following hair replacement, you can expect a few days of swelling and soreness which is well controlled with oral pain medications. Swelling generally subsides by 5 days. You can resume most normal activities after a few days, except for intense exercise, which may be resumed at about 3 weeks. 06FUE Hair transplant has become the new standard in hair restoration over the last few years in the field of hair transplantation. Hair Transplantation by Follicular Unit Extraction (FUE) uses hair graft follicles for hair transplants each follicle consists of one to four hair terminal.

 

Thel hair follicles consists of sebaceous (oil) glands, a small muscle, tiny nerves and small blood vessels. Every follicle is covered by the surrounding sheath that is mainly made of a protein (collagen)

 

A follicle is not only an anatomic but a physiologic one. Preserving other important structures in a follicle helps that each hair functions normally and looks natural.  Follicular Unit Extraction – FUE

Follicular Unit Extraction (FUE) is the method of obtaining hair grafts directly and individually from the patient donor area. This process of collecting hair units was first introduced by Dr. Woods in Australia. Using different FUE techniques, we perform a few biopsies under local anesthesia and try to extract few follicular units with special biopsy devices. We examin the harvested grafts microscopically to make sure they are intact before proceding with a larger FUE or FIT session. The text is positive if we can remove intact follicular units without transecting them.

 

FUE Transplant with SAFE System

 

At Dr.Feroz Khan’s Darling Roots Hair Restoration, we continuously thrive to improve our techniques of hair restoration and provide better results for our patients. We perform  experiments on different motorized and manual techniques of Follicular Unit Extraction (FUE) in our Darling Roots hair transplant center. The result of

our study is in favor of SAFE system in comparison to other commercially available techniques for FUE procedure. Since then we have been using SAFE System in most our FUE surgeries.

 

Powered SAFE System for FUE is a relatively new device for minimally invasive FUE procedures. SAFE system has been proven to have lesser transaction rate (cutting intact hair follicles below the skin level) so a higher yield of hair follicles could be expected from FUE transplants with SAFE method. SAFE hair restoration system uses a blunt rather than sharp punch, which is used in most other FUE devices. Not having a sharp punch is an important factor that minimizes the damage to the hair follicles. Thus more intact follicular units are expected to be harvested in every hair transplant

Follicular Unit Extraction (FUE) is also called Follicular Isolation Technique (FIT). The advantage of FUE is that patients will not have a linear scar on the back of their head, minimal pain and blood loss and recover the next day.

 

In FUE or FIT, we do not remove the strip of skin from the donor area. Patients will not have donor complications seen with the strip technique such as discomfort in donor area after surgery, the need for staple or suture removal and presence of linear scar.

FUE is the procedure of choice for people who want style their hair short to medium in their back. It is also first choice for those who have previous healing problems with any scalp procedure.

Follicular Hair Transplant: The natural results

Follicular hair transplant is becoming the standard hair transplant procedure because of its natural look and functionality. One reason for its natural look is due to the fact that the surgeon has more control on the direction of the follicular grafts. Hair transplant surgeon can place follicular units in the exact natural direction and distribution.

 

In comparison, old techniques of micro-graft, mini-graft or plugs leaves surgeons very little to no control on the distribution and direction of each follicular unit and the way the hair units are located in the donor area dictates their final direction after hair transplant.

 

Hair follicles are transplanted  to recipient sites in the same manner that hairs grow naturally. In other words, the natural grouping of hairs into follicular units is preserved. The procedure usually involves local anesthesia with sedation. The duration of the procedures ranges from a couple to several hours and depends upon the number of grafts transplanted.

Results
After approximately six weeks, the transplanted hair will fall out and be replaced about three months later when the new hair grows in. Results are entirely apparent by 9-12 months. To achieve sufficient density, several sessions may be required. Transplantation is a progressive process requiring hundreds of micro-grafts, each of which will generally contain from one to three hairs grafted from a donor site located on back of the head.

  • Autoimmune disorders such as alopecia areata or lupus.

Eyebrow transplant is done through harvesting scalp hair from the back of the scalp, known as the donor area. The removed skin is dissected with microscopic magnification to follicular units. Hair can be harvested with conventional strip techniques or FUE. Dr.Feroz Khan’s Darling Roots Hair Restoration offers both techniques for harvesting donor hair for eyebrow hair transplant.

The area is prepared and very small incisions are made in the eyebrow area. Tiny incisions produce a tight grip for the single hair grafts that are already harvested. In order to create the best natural results, making the normal looking direction and distribution is very important.

 

The growth of transplanted hair in eyebrow follows the pattern of scalp hair growth. Transplanted eyebrow hairs are permanent and grow constantly needing to be trimmed more often than normal eyebrow hair. Patients may need 60 to 200 follicular hair grafts for each eyebrow. The number of hair grafts may vary based on patient’s sex and ethnic background and eventually individual facial features.

 

The procedure is performed using local anesthesia. Complications after this procedure are minimal. Redness, swelling and scabbing can be seen during the first few days after eyebrow surgery, but usually go away in 3 to 5 days. Daily washings will minimize scabbing and facilitate healing of implanted hair grafts. Newly transplanted hair to the eyebrow area usually falls out in the first 2 weeks, but new hair grow from those follicles in about 3 to 4 months. Full results are expected in about 6 months.

 

Body Hair Transplant

 

Some patients have limited or depleted donor hair available in these instances body hair may be a suitable source of donor hair.

 

Body hair can be harvested and utilized for scalp hair transplantation in people with hair loss who have both good course body hairs whose scalp donor area is severely depleted.  Individuals who have good quality hair in their permanent zone of DONOR area (back and sides of their head) should not have body hair transplanted. Utilizing body hair is an alternative form of donor hair for special cases. Since body hair has a short growth cycle in comparison to scalp hair, it is less desirable than scalp hair for transplantation; therefore only utilized in exceptional circumstance.

 

Growth Cycle Comparison

During the normal hair growth, cycle a hair grows 1 to 6 years. After this the hair then transitions into its resting phase for 4 to 6 weeks; the end of phase is noted by the shedding of the hair before the start of another growth cycle and growth of another hair.

 

The growth cycle of body hair consists of much shorter growth phase and a longer resting phase. In other words, many body hair follicles that you transplant stay in resting phase most of their lives and only a fraction of them grow and could be seen at any given period of time hence the body being less hairy than the scalp. When transplanting body hair follicles the client must be aware of this factor and understand that the results will be affected by this factor.

However, body hair transplants are OK to use for people who have no alternative i.e.  bad donor hair density, unfavorable hair quality tight donor areas with significant scarring that makes hair difficult to obtain for transplantation.

1.       Camouflage and repair the spotted balding areas.
Old plugs or large mini- and micro-grafts could be easily hidden by removing them and producing a natural hair appearance using follicular unit grafts. 2.       Repair of unnatural hair direction.
Unnatural hair direction particularly in frontal and hairline areas can be very disturbing for patients and causes great difficulties with hairstyling. This is not the case in patients with old technique surgery using large grafts. The direction of hair in these patients can be corrected by removing individual hair follicles with unnatural direction and replacing them with follicular unit grafts in natural direction and orientation.

3.       Repair of abnormal hairline.
Abnormal hairline is either due to pluggy appearance of the hairline or designing problems. Normal hairline is generally irregular with a transitional appearance in front and a densely packed area behind it. Orientation of normal hairline is so irregular that one should never be able to connect the dots to make a straight line. Transitional irregular hairline could be easily created by implanting refined follicular unit grafts giving a natural and undetectable result.

4.       Repair of scalp scars from scalp reduction, cranial surgeries and open donor.
Scar of any scalp procedures such as trauma or neurosurgical procedures can be easily filled with transplanting follicular units into the scars. Patients may need more than one procedure in order to increase the hair density in the scar area and to minimize the sharp contrast between the scar and the hairy area of the scalp around the scar.

Washing Hair after Hair Transplant

The first 4 days after a hair transplant are the most critical as hair transplant patients need to be extra careful about washing their hair, especially in the transplanted areas. Instructions will be given on how to properly wash, with critical care of the transplanted areas and should be followed stricyly as instructed by Dr.Feroz Khan or his Darling Roots staff. After 7 days most people can return to a normal hair washing routine.

Returning to Work after Hair Transplant

You can return to work as early as the day after your hair restoration procedure, but if your job requires any kind of lifting, bending, stretching, physical strain or touching chemicals, patients can take a few days off until the acute phase is over. Some patients may want to take days off only because they want to keep their hair transplant procedure confidential. In cases like this patients can take 1 to 2 weeks off from work. The most critical time for patients after the hair transplant procedure are the first four days, as they must be extra careful to avoid any type of strain, after four days the transplanted grafts are usually well settled.

Physical Activities after Hair Transplant

There are no limitations for mild exercises as long as the patient does not bend his/her head aggressively or touch the transplanted area. Contact sports should be avoided for the first month in most patients who had a transplant surgery.

Possible Complications

Pain and Discomfort: There may be some aching in the donor area, which starts within the first few hours after hair transplantation. Pain medicine is generally provided to alleviate the discomfort during the first night. After the first night, pain should be well controlled. Most patients do not need any medications for pain after the first night, but if there is any remaining pain during the second day, mild pain medications such as Tylenol, in most cases, should be enough to make you feel more comfortable.

Swelling: It is common for the first four days after a hair restoration procedure for swelling to occur around the transplanted area. Swelling is due to the process of healing and may range from non-existent to severe in which the subcutaneous fluid descends from the forehead area to around the eyes. The maximum swelling is generally seen in days 3 and 4 after hair transplantation. Patients may need steroids to prevent swelling after their hair restoration. It is good to sleep in a semi-sitting position and keep the upper body elevated to help minimize the swelling, or shorten its duration.

 

Bruising: Bruising or discoloration of skin around the transplanted area might be present for the first few days after the hair restoration procedure. This may involve the recipient area, and at times, may extend to the forehead.

Scabbing: Scabs should flake off within one week if the patient properly washes the transplanted areas. We recommend washing transplanted areas, the way we show it, twice a day for the first 4 days. Patients can go back to their routine hair wash after that time unless they continue to have scabbing. Those who have scabbing will need to continue washing their head twice a day, until the scabbing goes away.

 

Itching: Itching is a common symptom after any type of wound healing. Hair transplantation is not an exception to that reality. Patients may feel itching on transplanted and donor areas for the first few days. This may extend to the second week after hair transplant, but should be mild and does not require any treatment for most patients. If itching is significant, a prescription of a mild steroid cream might help.

 

2 Weeks after FUE Hair Transplant

Physical Activity

It’s okay to resume exercise 7 days after hair transplant. It is critical to be selective about the types of exercises, which can be done (such as not doing squats, bend, lifts, etc.) for the first seven days after surgery. Always aim to keep your head elevated/upright. Bending the head may increase the tension of both the donor and the recipient area which may increase the chance of stretching the grafted area.

There are no possible Complications except Inflammatory redness.

Redness and Inflammation: The inflammation and redness should be subsided at this time, but a mild pinkish discoloration of the transplanted area may remain. This can be visible for a few weeks comparable to a sun burn, especially in patients with lighter skin.

 

4 Weeks after Hair Transplant

Hair Growth

Transplanted hair grafts should have set in their new location on the scalp and the hairs attached should have fallen out. After being replanted, the stubby hair falls out and the new strands of hair begin to grow after a couple of months.

Physical Activity

Patients should be able to resume most physical activities they used to do before hair transplantation. The key is avoiding strain on the scar area if a strip, or FUT, hair transplant was performed. If an FUE procedure was performed, then more intense exercises are permitted.

3 Months After Hair Transplant

Appearance

Patient’s appearance all Outlook 360 should be the same as before having the hair transplantation done.. Patients generally do not keep the newly transplanted hair, and commonly lose the shaft of hairs while the follicles become part of the scalp and generate new hairs.

New Growth

Patients who have complete baldness (no visible hair before hair transplant), should be able to see some very small hair in the transplanted area. Folliculitis or pimple shaped lesions may be noted in some patients at this time. They generally form because of the collection of sebum (oily secretions of the hair follicles). Folliculitis lesions usually drain spontaneously without any need for any interventions. If folliculitis lesions are more than two or they are painful, you need to see your surgeon to have those lesions drained for you. Drainage of folliculitis is generally simple and painless. The stuff from the lesion will be squeezed out by making a small prick using a small sterile needle.

Possible Complication

Shock Loss: This may occur for some patients after their hair transplantation. Patients may lose some of their native hair due to shock loss after their hair transplant. This happens while they still do not see the bulk of the transplanted hair grown in yet. Some patients may feel the bulk of their hair is lessening more than what it was before the hair transplant. This does happen at times until the next few months after hair transplant when the patients can see the newly transplanted hair along with regrowth from some of the hair that has gone through shock loss.

Hair Growth

For three-months appointments are recommended monthly or as decided by Dr.Feroz Khan to all patients after a hair transplant in order to be seen for an early evaluation of their growth. This is a very exciting time for patients, as they may begin to see   first signs of growth for the transplanted hair. Although, some patients will begin to see their full results at six months, most patients will continue to see further improvement after month 6.

Postoperative six-month visit is important for some patients because it may determine if they need to consider another hair transplant.  Patients who wish to achieve very high density of hair along their hairline or other areas may need a second procedure in order to double up the density of hair to those areas.  We generally cannot implant more hair until SIX  months past a previously transplanted area for three reasons:

  1.  We do not have a solid understanding of the final growth before this time.
  2.  Some of the transplanted hair has not grown before the six-month mark so there is a risk that we may transplant grafts on top of previously transplanted area.
  3.   The donor scar is not quite mature and the laxity of the scalp might not be completely back to the normal.

 

1 Year After Hair Transplant

The results from the hair restoration procedure should be obvious and noticeable. At this time the new hair will have grown to its normal thickness. Although most patients may see most of their results at this time, more improvement in fullness and thickness should be expected up until 18 months from the hair transplant.

Scalp Micro Pigmentation (SMP)

Scalp Micro Pigmentation (SMP) is one of the newer cosmetic procedure that is offered in Dr.Feroz Khan’s Darling Roots / Cosmetic Outlook Clinic. SMP utilizes dermal pigments that match the patient’s hair color. These pigments are introduced into the scalp improving the appearance of the balding areas. It may also be used for a variety of other purposes. A SMP procedure can be done with or without local anesthesia. This depends on the location and size of the area that needs micro pigmentation.

When performing a Scalp Micro Pigmentation procedure we utilize very fine needles to induce the pigments into the superficial layers of the skin and below the epidermis. The most common indication of COSMETIC micro pigmentation is to create the shade of hair similar to what could be seen in a shaved  Many men shave their head to hide their male patterned baldness. However, the lack of the shadow from shaved hair shafts makes balding apparent on sight. Utilizing SMP, we can re-create the shade of hair in the balding areas. Recovery after Hair Transplants
Following hair replacement, you can expect a few days of swelling and soreness which is well controlled with oral pain medications. Swelling generally subsides by 5 days. Any sutures at the donor site are typically removed by day 7. You can resume most normal activities after a few days, except for intense exercise, which may be resumed at about 3 weeks. Most people are able to return to work within days, but may choose to return in a week. The donor site scar is concealed by the hair. Recipient sites are usually not noticeable after about a week.
